Albany MS athletic opportunities are quickly approaching! Make sure you get the start dates on your calendars & eligibility paperwork completed for your athlete to be eligible to participate. If you prefer hard copies of the paperwork, they can be found in the district office with Mrs. Seavert. The AED/CPR/Concussion Form and Code of Conduct Form are completed with school registration. Attached is a PDF of the WIAA Physical & Alternate Year Card.
ELIGIBILITY PAPERWORK NEEDED
1) WIAA Physical or Alternate Year Card ( A physical is needed every other year)
2) Albany AED/CPR/Concussion Form (Participant & Parent/Guardian Form) (Completed online with student registration)
3) Albany HS Code of Conduct Form (Completed online with student registration)
4) $20.00 MS Athletic Fee (Can pay online via infinite campus or to the office via cash/check. Isn't payable online via infinite campus until after the start date of the season)
***A WIAA physical is REQUIRED every two years. The opposite year of a physical is an alternate year card.
