Athletics Fees

Fall athletic opportunities for Albany MS/HS students will be starting in August. Make sure that you are getting a head-start with the things needed for your student-athlete to be eligible to participate in Albany athletics. Online athletic forms can be found on the Albany School District website or hard copies can be found in the district office. Items listed below are what will be needed for student-athletes this Fall.

  • Albany MS/HS Fall Athletics Eligibility Information

  • WIAA Physical Form or WIAA Alternate Year Card Form

  • Students need one physical every two years. An alternate year card must be filled out years opposite of a physical.

  • Albany Emergency Contact Form

  • WIAA Concussion Form – Parent & Athlete

  • Albany MS/HS Extra-Curricular Code of Conduct Contract

    • (Coincides with Extra-Curricular Handbook)

***If a MS/HS student-athlete is participating in one of our co-op sports they will need to have the Albany Coop Form completed as well. (Parkview-Albany Wrestling, Parkview-Albany Football, Juda-Albany Softball, Monticello-Albany Baseball)

Participation Fees (to be turned in to coach)

  • Middle School $20.00 per activity

  • High School $40.00 per activity

Season Passes (Albany only events)

  • $20.00 Student (Albany HS Students show student ID to ticket taker for free admission to Albany home events)

  • $40.00 Adult

  • $75.00 Family
